Configurator: Can not configure server connection (AD Authentication)

When trying to configure a connection to a local Dynamics CRM deployment using AD Authentication in "CRM Queue Manager Configurator" the following issues occur during "Step 2":
a) Label for field "username" overlaps with field input area - need to type username blindly (grows to "Username [domain\username]" after specifying the server URL)
b) The Password is readable (should be dotted)
c) An exception message is displayed "Object reference not set to an instance of an object". Even after trying with different credentials etc.

I have attached a new version of CRM Queuemanager Configuration tool.
This version creates a log if anything goes wrong.
Could you give it a try and send me the log if you receive an error?
No connectioninformation will be logged, only stack trace.

I also fixed the overlaps problem and the password is now hidden.

I tried the new version. Issues a) and b) fixed now :-)

I think I found the problem but unfortently I can't test it since we don't have an on-premisis CRM installation.

I have attached a new version to my post.
Unzip if and overwrite your current existing files with the files inside this package.

Let me know how it goes.
